Our home care specialty programs have been developed to assist patients, families and physicians treat a variety of diseases and conditions in the home environment.
We provide a range of home care services including senior care, nursing, physical, occupational, and speech therapy. In addition, our team of health care professionals has worked in conjunction with physicians to develop specialty programs to assist patients with managing conditions, surgeries, and diseases at home.
Often these specialty programs are an appealing alternative to “facility care” for patients as well as referral sources and payers because patients can avoid the hospital or stay there for the shortest period possible.

Joint Replacement & Orthopedic Recovery
Orthopedic surgeons rely on our specialized care to help patients recover swiftly after hip or knee replacements. Following evidence-based ortho protocols, our team ensures a seamless recovery process:
1. RN Evaluation – A registered nurse conducts an in-home assessment, reviews surgical incisions, addresses pain management, and identifies safety modifications.
2. Timely Therapy – Within 48 hours, physical therapy begins with a personalized care plan.
3. Ongoing Support – Over the next 2–3 weeks, therapists visit 1–2 times weekly, tracking progress and coordinating with surgeons.
4. Transition Planning – We prepare patients for outpatient therapy, even scheduling their first appointment.
We also manage arthritis, fractures, osteoporosis, and other orthopedic conditions with tailored therapeutic interventions.

Dementia & Alzheimer’s Care
Led by a Teepa Snow-certified Director of Nursing, our staff employs the Positive Approach to Brain Change™ to support dementia patients. We focus on:
● Understanding disease progression and behavioral cues.
● Using gentle, empathetic communication techniques.
● Creating a calm environment that reduces agitation.

Fall Prevention & Home Safety
Reducing fall risks is critical to maintaining independence. Our team:
● Conducts home safety evaluations (lighting, clutter, grab bars).
● Reviews medications for dizziness or balance side effects.
● Provides gait and balance training through physical therapy.
● Recommends adaptive equipment to enhance mobility confidence.
